Seems that the folks at Reuters can't tell a skin head from the guy who yelled meat head. This June 25 wire service story announcing the planned marriage of Sinead O'Connor was accompanied by a photo of the freshly deceased Carroll O'Connor. TSG's happy to report the error was corrected in a brisk six hours.(1 page)
